It is the Medal of Honor. It is not the Congressional Medal of Honor. The link below is to the Center for Military History. You can read all the certificates for the MOH. Click on the Iraq link for the most recent.
Edit: Andrew should not be allowed out in polite society. He is also completely wrong as the very site he references states. The Congressional Medal of Honor Society is a society created by Congress. What do you expect from them. Read the opening lines from that site: (link pasted below as well) The CMOHS clearly states that the medal is the Medal of Honor.
